On the form you will provide the following:

• Your name as you attended Scripps and current legal name
• Year of graduation
• Birthdate
• Provide your email address and phone number
• Address to where you wish to have the diploma sent when ready

The fee is $75.00, so make a check or money order payable in this amount to swag外流 and include with your completed form. Additional fees may incur for shipping international or via FedEx.

Replacement orders may take between 6-8 weeks for processing.
